Property Damage Report -- July 2021 Storm

The Town is collecting data from Sand Lake residents on property damage due to flooding following the storm on July 14, 2021.

Residents who have experienced property damage should complete the form below to provide their name, property address, contact information, description and dollar amount of damage (even if it is your best estimate) to the Town. 

This information will be aggregated to provided to federal, state and county disaster relief agencies. 

This form is for Sand Lake property damage only (hamlets of West Sand Lake, Averill Park, and Sand Lake) please.
